Terms and Conditions:

• I give permission for the child to play cricket for the Greytown Junior Cricket Club for the 2018 season.
• I agree to pay the subscription fee of $45 by 28 Feb 2018.
• My child understands the standard of behaviour expected and I accept that my child may be removed from a practice, game or team if their behaviour does not meet these expectations.
• I consent to the collection of the above details for use by the Greytown Cricket Club. By accepting this agreement I acknowledge that these completed details will be input into the CricHQ database and system as the official partner and database of New Zealand Cricket. I provide my consent to the use of these details for census purposes and data collation and allow access to these details by the Wairarapa Cricket Association, Central Districts Cricket and New Zealand Cricket. It is agreed that these details will not be passed to a Third Party not named in this declaration, nor will private personal details be shared or published by any of these parties.
• If my child is selected for representation at Wairarapa level or for a Major Association (Central Districts) we also consent that data entered into CricHQ can be utilised by these parties for any use pertaining to the playing, selection and analysis of that team or Association.
• I acknowledge my right to access and correct this information. This consent is given in accordance with the Privacy Act 1993.
